Supplementary Privacy Notice
Durham County Council have commissioned a new collaborative approach to a range of support services including prevention, early identification and recovery support for people who are experiencing mental health issues. This collaborative approach is known as an Alliance.
Durham Mental Wellbeing Alliance (DMWA) will act as a Single Point of Access for all providers and sub-contractors, they will be responsible for signposting individuals to support providers, collating information and reporting on the overall performance of the Alliance.

This Privacy Notice is supplementary to the individual Privacy Policies/notices of each contributing alliance partner and aims to provide more information and links to further resources in order to comply with the General Data Protection Regulation and other applicable UK data protection law, This Privacy Statement is applicable to your initial referral to the Durham Mental Wellbeing Alliance. Once your referral has been made/transferred to one of the alliances partners the protection of your data will be governed under that partners privacy policy. Why do we need to collect your personal information?
To help us deliver efficient services and to manage your relationship with us, we need to collect relevant personal details from you. Your personal information will only ever be used in order to assess your needs and signpost you to the appropriate support providers. We will also collate aggregated data such as statistical or demographic data which does not identify you individually and report on the overall performance of the Alliance to our commissioner. In this instance our commissioner is Durham County Council. For added transparency it is Home Group Ltd who is coordinating this initial referral function this is team is known as the Single Point of Access Team. Your personal data will always be processed in accordance with the law.
What Information we collect about you?
The personal information you have provided, we have collected from you, or we have received about you from third parties may include:
- name, address and address history, date of birth, and gender (and proof of your identity/photo ID) your National Insurance Number
- contact details, including telephone number and/or email address
- details about your family and dependents (e.g., your marital status and number of children) and anyone authorised to act on your behalf if applicable
- health details and medical history
- information about disabilities or vulnerabilities
- details about your race, ethnicity, religious beliefs, sex life, sexual orientation
- other personal information required to enable us to provide you with the service or support you have requested
- aggregated data such as statistical or demographic data which does not identify you individually
How we collect information about you?
We may collect information about you in the following ways
- when you apply to make a referral to the Durham Mental Wellbeing Alliance via our Hard Copy (paper) application form or our online application form via our website
- when we receive information about you from third parties such as your existing support provider
- when you make a complaint or provide feedback
- when you contact us on social media
- when we take photographs at our events, at our properties and in our communities for general publicity use (we will only use photographs in which you can be identified if you have given us your written consent)
- when you call, write, email, text or meet with us
- automated technologies or interactions – when you interact with our website, we may automatically collect technical data about your equipment, browsing actions and patterns. We collect this personal – information from you by using cookies and other similar technologies

Who we may share your personal information with?
Personal information will only be shared by DMWA to Providers and Subcontractors within the Alliance. Statistical data will also be shared with Durham County Council, Alliance partners and Alliance sub-contractors. Statistical data may also be published to demonstrate the overall performance of the Alliance.
In extreme circumstances and only ever where relevant, given the nature of the services provided to you, we may also need or have a legal obligation to share your information with the following categories of third parties:
- doctors, mental health teams
- next of kin, family members
- local authorities,
- social services
- the police and other emergency services
- government agencies and departments and regulatory bodies
- commissioners
How will your information be kept secure?
We are committed to ensuring that your information is kept secure. In order to prevent unauthorised access or disclosure, we have put in place suitable physical, electronic and operational procedures to safeguard and secure the information we collect online and in person.
Processing your information outside of the EEA (European Economic Area) We have put in place procedures to deal with any suspected personal data breach and will notify you and any applicable regulator of a breach where we are legally required to do so. Once your referral has been made to the relevant provider, we would suggest you check that providers own Privacy Policy to see what their approach is to security.
We will not transfer or store your personal information outside of the European Economic Area (EEA). Once your referral has been made to the relevant provider, we would suggest you check that providers own Privacy Policy to see what their approach is to information processing.
What if you choose not to give personal information?
We may need to collect certain personal information by law, or under the terms of a contract we have with you.
If you choose not to give us this personal information, it may in certain limited circumstances delay or prevent us from meeting our obligations to you. It may also mean that we cannot perform the services you have asked for. In some circumstances, it could mean that we will need to terminate our relationship with you.
We will always tell you at the time we try to collect such personal data, if your refusal to provide that data may impact on our ability to provide services to you.
How Long will your personal information be kept?
We will only keep your information for as long as necessary to fulfil the purpose we collected it for, including for the purposes of satisfying any legal or reporting requirements and in accordance with our retention and disposal guidelines. Once your referral has been made to the relevant provider, we would suggest you check that providers own Privacy Policy to see what their approach is to data retention.
What are your rights?
Under the legislation you have the following rights in respect of your personal information:
- to obtain access to, and copies of, the personal information we hold about you i.e. to make a Subject Access Request (SAR)
- to require that we cease processing your personal information if the processing is causing you damage or distress
- where we rely on your consent to process your personal information, you can withdraw your consent at any time by contacting your relevant provider
- to require us not to send you marketing communications
- to require us to erase your personal information
- to require us to restrict or for you to object to our data processing activities
- to receive from us the personal information we hold about you which you have provided to us in a reasonable format specified by you, including for the purposes of you transmitting that personal information to another data controller (right to data portability)
- to require us to correct the personal information we hold about you if it is incorrect
Please note that these rights may be limited by the legislation, and we may be entitled to refuse requests where exceptions apply. You can find out more about your rights from the Information Commissioner’s Office.
Should you wish to exercise any of your rights, please contact your support provider directly.
What will happen next?
Your support services will continue as agreed by your provider, they will ask you to consent to sharing information with DMWA, Home Group Ltd will receive your information on behalf of DMWA and this will be processed in accordance with the law.
